One CHW will focus on the Latino community in South Richmond and one on East Richmond and Henrico. These positions are full time roles funded through a 4 year VDH grant. They will focus on blood pressure management, heart health educational classes and navigating people to primary care.
These incumbents will be expected to work in our Richmond Health District (400 East Cary St) as well as other offsite areas in the community as needed. Mileage reimbursement is available for any work-related travel. Typical work hours are Monday - Friday 8:00 am - 5:00 pm.
